Anchor Health provides relationship-based telehealth primary care across Maryland across five clinical pillars. Referring providers can send patients for whole-person primary care or for a specific pillar — whichever best fits the patient’s needs.

Whether your patient needs a long-term primary care home or a targeted intervention in one pillar, Dr. Joseph builds a continuity-based relationship with every patient — the same provider, every visit.

The Referral Process

A simple handoff in three steps

Refer
Send the patient via secure email to anchorhealthos@polsia.app. Include the patient’s name, contact information, reason for referral, and any relevant clinical context or records. The intake is structured to be quick and HIPAA-aware — we’ll handle the coordination from there.

We Contact
Dr. Joseph’s team reaches the patient within two business days. We schedule the new-patient visit, confirm Maryland residency and insurance, gather any history we need before the appointment, and provide the patient with what to expect from their first Anchor Health telehealth visit.

Shared Care
Visit notes are sent back to the referring clinician with the patient’s consent. We coordinate bidirectionally on labs, prescriptions, follow-ups, and any specialty escalations — so your patient experiences one continuous care team, not two disconnected practices.
