Who we serve

Anchor Health provides relationship-based telehealth primary care across Maryland across five clinical pillars. Referring providers can send patients for whole-person primary care or for a specific pillar — whichever best fits the patient’s needs.

Family Medicine & Primary Care — annual wellness exams, chronic disease management, acute illness, preventive screenings, and care coordination.
Women’s Health — perimenopause, PCOS, hormone health, gynecology, and reproductive care across the lifespan.
Weight & Metabolic Health — evidence-based weight management including GLP-1 medications, metabolic syndrome care, nutrition counseling.
Mental Health — integrated mental health support within primary care: depression, anxiety, ADHD screening, stress, sleep.
Pediatric & Adolescent Care — well-child visits, adolescent health, school and sports physicals, developmental guidance.

Whether your patient needs a long-term primary care home or a targeted intervention in one pillar, Dr. Joseph builds a continuity-based relationship with every patient — the same provider, every visit.

The Referral Process

A simple handoff in three steps

Refer

Send the patient via secure email to anchorhealthos@polsia.app. Include the patient’s name, contact information, reason for referral, and any relevant clinical context or records. The intake is structured to be quick and HIPAA-aware — we’ll handle the coordination from there.

We Contact

Dr. Joseph’s team reaches the patient within two business days. We schedule the new-patient visit, confirm Maryland residency and insurance, gather any history we need before the appointment, and provide the patient with what to expect from their first Anchor Health telehealth visit.

Shared Care

Visit notes are sent back to the referring clinician with the patient’s consent. We coordinate bidirectionally on labs, prescriptions, follow-ups, and any specialty escalations — so your patient experiences one continuous care team, not two disconnected practices.

A direct line to Dr. Joseph for clinical questions on shared patients. Async messaging is available for established patients between visits, so your team gets responsive answers without phone tag — and we’re reachable for urgent coordination when a patient is in active care.

Shared Visit Notes

Every visit summary is forwarded to the referring clinician on request, with the patient’s consent. You see what we diagnosed, what we prescribed, what labs we ordered, and what follow-up we scheduled — so your records stay complete and your care plan stays aligned.
